If you’re a patient
Choose the self-referral visit type in the form below and answer a few questions about what brings you in. You don’t need a doctor’s referral to be seen — you’re welcome to refer yourself.
If you’re a referring provider
Choose the provider referral visit type. You’ll be asked for your details and your patient’s, along with the reason for referral. Requests are reviewed and our office contacts the patient to schedule.
A note about LENS Neurofeedback
Neurofeedback works best when we give the brain regular reminders that we are asking it to change. We ask that you be able to commit to the first 6 weeks of neurofeedback as close to weekly as you can get. Please submit your request and we will reach out to you to schedule your first 6 appointments. At the 6th appointment we will evaluate your response to treatment and reassess next steps.
